Insurance
We accept most insurance plans.  You are responsible for knowing your insurance policy.  In order for us to bill your insurance correctly, please have available the insurance card, subscriber name and subscriber date of birth and copayment.  We do ask for this information at every appointment as insurance information can change frequently.

Charges

Unless previous arrangements have been made, payment is due at the time of service.  We do accept Visa, Mastercard, American Express and Discover, Cash and Check.  Copayments are a contractual obligation between you and your insurance company.  Copayments are expected at time of service.

DEDCUCTIBLES AND CO-INSURANCE

The goal of our office is to deliver the best and most prudent medical care to our patients.  If testing and/or procedures are recommended, or it has been determined by your provider that they are a necessary part of your care, these services will be submitted to your medical insurance.  These services may be applied to your deductible and/or co-insurance.

No-shows

In order to provide timely care to our patients, we ask that you would call us if you are unable to keep your appointment. Beginning February 1st, 2013 a no-show fee of $50 will be charged (this is not covered by insurance).

Telemedicine Billing Policy

Our practice is using telemedicine when appropriate to facilitate patient care.  Telemedicine visits may be scheduled in advance and will be done either with a video connection or telephonically.  Not all conditions and circumstances are appropriate for telemedicine and the office will work with you to arrange the appropriate visit type.

We often perform follow-up work via telehealth, telephone and even the patient portal.  This can include follow up on lab work, appropriate follow up from preventative medicine screening, and other circumstances.  This is done to make the process easier and more streamlined for you, rather than requiring an in-person visit when appropriate.

After hours phone calls may result in a documented telemedicine visit if the on-call provider deems doing so is appropriate for proper patient care.

For all visits (both in-person and telemedicine) we submit billing to your insurance provider, and you may be responsible for a portion of that charge.  These charges may include a copay, or charges related to a deductible or other provisions of your specific insurance product. These charges are based on the agreement you have with your insurance provider and not dictated by our practice. 

UNINSURED PATIENTS

If you do not have insurance, you will be required to pay $50.00 at the time of visit.  You will be billed for the balance and any additional costs incurred for the visit.  If you need assistance with an alternative payment plan please contact our billing staff to make arrangements.

 ACCOUNT BALANCES

Whether or not you have insurance or are self-pay, payment of any account balance is expected within 30 days of receipt of your billing statement.  If you need special arrangements for payment, you may contact our billing staff to make mutually agreeable alternative payments.

 RETURNED CHECKS

A $30.00 fee will be assessed for any returned check (this fee is subject to change according to changes in fees assessed by the bank) in addition to the amount of payment for that check.

 COPY OR TRANSFER OF MEDICAL RECORDS

A $15.00 fee is charged per family for copies or transfer of medical records.
